Year 8 HPV vaccination is NOW DUE. This is free from the NHS. Please see the email previously sent for the consent form and information about the vaccine. Access to the consent forms will close two working days prior to the session. Consent closes: 9 am Monday 15 April 2024
If you decide you do not wish your child to receive these vaccinations, please submit the form and indicate that you do not consent. This will enable us to update your child’s immunisation record.
