This is a reminder that the School Age Immunisation Service is due to carry out the HPV immunisation session at Perins on Wednesday 17 April 2024 for Years 8-11.
If you have NOT already completed the consent form, please complete the below. Only complete one consent form per child. This is free from the NHS.
Students having the vaccine, must make sure they can roll up their sleeves and have the means to access food at break and lunch, and a filled water bottle.

If you decide you do not wish your child to receive the vaccine, please still submit the form and indicate that you do not consent. This will enable us to update your child’s immunisation record.
All forms completed before 30 April 2024 will be entered into a prize draw to win a £100 voucher to spend at a local supermarket.
