As requested by The NHS School Age Immunisation Team, at the beginning of this week we distributed paper consent forms to students for Covid-19 vaccinations*. If you haven’t received one for your child please contact school@perins.hants.sch.uk
Please complete the relevant part of the form to either give consent or to decline consent for the Covid-19 vaccine.
The Immunisation team have rescheduled the flu immunisations at school to Thursday 7th and Friday 8th October when they will also be administering the Covid-19 vaccination to 12–15-year-olds students who have consent. The link to the Flu vaccination consent has remained open as the visit date has been extended.
Please be aware that we are passing-on all information we receive. Our staff are not able to answer questions on the vaccination programmes.
